Participant's most recent benefit statement or an account balance for each plan
listed below;

4.
|
Pleadings
which set forth the designation of the court, name of the judge, case and
docket number, name, address and telephone number of attorneys; and
|
5.
|
Portion of
the judgment or settlement agreement dividing each plan listed below.

| Note: The plan
administrator may require written authorization from the participant
authorizing the release of relevant plan information to assist in the
preparation of the QDRO. A sample participant
authorization is posted on this website.
